What to gather and think about before your first conversation with a care team.

A little preparation helps your first consultation go smoothly and helps the care team understand what matters most.
Make a list of daily tasks that have become difficult, any recent changes in health or routine, medications being taken, and any safety concerns in the home.
Note the person's preferences — favorite routines, foods, activities, and anything that brings comfort. Think about timing: when support is most needed and how soon you'd like it to begin.
Write down your own questions. The first conversation is as much about you understanding Access as it is about Access understanding your situation.
