Guide for Professional Referral Partners

Referral partners — hospitals, doctors, case managers, social workers, rehab professionals, hospice providers, senior communities, and community organizations — play a vital role in connecting people to the right support.

To refer a client, use the Refer a Client form or call Access directly. Share general support needs and a preferred follow-up method. Please do not submit sensitive medical information through the public form.

The Access team will follow up to confirm whether the client's needs fit available services and to coordinate the safest next step, including any secure intake workflow if needed.
